Forms and Legal Information
Celebration requires that all guests fill out our required forms and turn them in before departure day. These forms include:
•Release of Liability
•Emergency Contact Form
•Medical Release Forms
•Medication List
•Covid-19 Form

Travel Insurance
Celebration requires that all guests have TRAVEL INSURANCE that meet our medical and travel requirements.
If you purchase your policy through Celebration, we will take care of the purchase and file any and all
claims. Guests may purchase their own TRAVEL INSURANCE, but will be responsible for purchasing and
filing any claims.

Travel Guard Preferred Plan
• Trip Cancellation: Up to 100% Insured Trip Cost
• Trip Interruption: Up to 150% Insured Trip Cost
• Travel Medical Expense/Dental Expense: Up to $50,000
• Emergency Evacuation and Repatriation of Remains: Up to $500,000
• Accidental Death & Dismemberment: Up to $30,000
• Pre-Existing Condition Waiver - Insurance MUST BE PURCHASED within 15 days of deposit
• Insurance pricing is determined by your AGE and PRICE of your trip
